
Advances in Assessing and Addressing the Needs of Family Caregivers

In this presentation we will discuss the development, testing, and implementation of the Preparedness Assessment for the Transition Home (PATH) instrument. Two speakers will share their respective implementation, results, and challenges. Exploration of the PATH tool in caregivers of patients with a glioblastoma will be shared.

- Examine the global state-of-the-art and science in assessing and addressing the needs of family caregivers using the Preparedness Assessment for the Transition Home (PATH) tool.

Dr. Barbara Lutz served as the McNeill Distinguished Professor at the University of North Carolina-Wilmington from 2013-2025 and is Associate Professor, Emeritus at the University of Florida, College of Nursing. Her career as a rehabilitation and public health nurse spans 40+ years. She has conducted rehabilitation nursing research related focusing on assessing and addressing the needs of stroke survivors and their family caregivers as they transition home. She has received funding from the Rehabilitation Nursing Foundation, Sigma Theta Tau International, National Institute of Nursing Research, and Patient-Centered Outcomes Research Institute. She served as President of ARN from 2019-2020 and has served on multiple committees for ARN and the American Heart Association/American Stroke Association (AHA/ASA). Dr. Lutz is a co-author on several AHA/ASA scientific statements including palliative care in stroke, risk adjustment for stroke, and best evidence on stroke caregiver interventions. She has served on several national committees and advisory boards including the ANA Care Coordination Expert Panel, Joint Commission Technical Advisory Panel for Comprehensive Stroke Center Certification, and National Center for Medical Rehabilitation Research Advisory Board. Dr. Lutz has a BSN from the University of Iowa, MS from Northern Illinois University, and MSN and PhD from the University of Wisconsin-Madison. Amy Ragnone’ s nursing career began after graduating from the University of Cincinnati with her associate degree in 1985. While working full time earned her BSN from UC in 1993. Amy served in the military as a Navy Nurse Corps Officer for 10 years. During her military service she was awarded 2 Navy Achievement medals as well as a Navy Commendation Medal for her service as well as her MA in Management from National University. After her military service ended, she worked as a manager/ Interim Director for Summerlin Hospital in Las Vegas, Nevada. In 2005 Amy took a faculty position at the College of Southern Nevada, earned her MSN from the University of Phoenix, and became a tenured Professor in 2009. She was active with the Nevada Nurses Association and served as the Vice President of the Nevada Nurses Association from 2008- 2010. In 2011 Amy and her children move to Ohio and in 2016 joined the Mercy Health system. She earned her DNP from Northern Kentucky University in 2022 and is currently the clinical nurse manager for the Acute Rehab Unit at The Jewish Hospital in Cincinnati Ohio.
