Breaking the Transfer Cycle: Innovative Strategies to Reduce Acute Care Transfers

This presentation highlights strategies that have successfully recused avoidable acute care transfers through proactive staff education, early intervention, and enhancing monitoring. The implementation of the ACT watch program empowers the nurses to recognize early warning signs and take preventative action. By fostering a culture of accountability and clinical excellence, our approach has led to improved patient care outcomes and overall operational efficiency. 

 

Learning Objectives

  • Analyze: Participants will analyze the root causes of acute care transfers in rehabilitation settings by reviewing case examples and identifying key contributing factors.
  • Apply: Participants will apply the methodology to simulate prioritizing high-risk patients, demonstrating their ability to implement targeted interventions in their own settings.
  • Evaluate: Participants will evaluate the effectiveness of interdisciplinary collaboration strategies by discussing their potential impact on critical thinking and patient outcomes in a post-presentation activity.
  • Upon completion participants will be able to implement evidence based strategies to reduce ACTs by identifying key risk factors, applying targeted interventions, improve critical thinking & interdiciplinary collaboration to improve patient care outcomes.
